Eastern Ontario Network (EON)




The Eastern Ontario Network (EON) is the PBLRN based out of Queen’s University’s Department of Family Medicine and one of the founding seven networks forming POPLAR. EON is made up of over 150 primary care providers across Kingston and southeastern Ontario. Since 2008, EON has collected de-identified EMR data from approximately 250,000 patients for research and quality improvement projects.


EON works closely with researchers, health care providers, patients, administrators, and other partners to better:

  • understand the needs of patients,

  • find ways in which we can improve clinical practices , and

  • answer clinical questions and fill gaps in knowledge.


Recently, EON in collaboration with the Frontenac Lennox and Addington Ontario Health Team, was able to link data between hospitals and primary care clinics. By linking the data, patients with chronic obstructive pulmonary disorder (or COPD) with a higher risk of going to the Emergency Department were found. Patients at high risk were then chosen for extra clinical care by a special nurse trained in line with clinical guidelines. This project and others like it show how the data can help to directly improve patient care. Click here to link to the abstract.
